Aftercare and discharge planning is a critical part of inpatient treatment. Patients should have a plan for follow-up care in place before they leave the hospital. Discharge planning should include a plan for where patients are going after leaving the hospital; often, this includes transferring to an outpatient program or attending therapy sessions.
